I've forgotten my Username - What do I do?

If you have forgotten your username:

1. Select the Need help accessing your account link on the login screen:

   

2. The Account Recovery screen displays, select Forgotten Username .
3. You are prompted to enter your email address.
4. Enter the email address you used when registering for Patient Services and select Recover username. If your email address is used for more than one patient, for example, your children or someone you care for, you are prompted to enter your First Name, Last Name and Date of Birth and then select Recover username.
5. An email is sent to your email address which contains your username.
